I think I might have a serious issue with the tank, where the sight glass mount to the tank a bit came off of the edge.  I was cleaning away the calcium build up with my kitchen Scotch pad and a bit came off the bottom mount point.

Edit:  Cleaned up around the mount points with 2k & 3k Wet Sanding and new photos.
