I had this happen twice. I suspect the DHL forms are confusing. It's easy to misplace the address parts.
Not as confusing as calling them for help!
I had a package from the UK being sent this past week, DHL held it up in customs because it was addressed to N Severson instead of Nick Severson

I called for help, but US DHL could not find the reason why it was being held and couldn't do anything about it, so I had to contact the shipper and he had to call UK DHL to fix the problem.
I don't even want to begin to tell the story about shipping my Maxitrak Case through DHL!
